Distance Services Book Request Form

This form is for the use of current HGSE:

• Doctoral students (D3+) living more than 50 miles from Cambridge
• Urban Superintendents Program students who have completed their first year
• Faculty on leave

Use this form to request books owned by Gutman Library or Widener Library.

• Always check the HOLLIS Catalog before submitting a request. Books that are checked out should be recalled in the HOLLIS Catalog (use the "request" feature) before you complete the form below.
